Determining the most difficult English grammar question is subjective, as it depends on the learner's background and proficiency level. However, one commonly challenging area is the correct use of subjunctive mood. 

For example:

Question: How would you rewrite the following sentence to correctly use the subjunctive mood?

"If I was you, I would study more."

Correct Answer: "If I were you, I would study more."

This change from "was" to "were" is a common area where learners make mistakes.
