Apart from reading English books or newspapers and watching English movies and TV series, the best way to improve is to practice. No matter how much you read or watch English movies, until and unless you don't start speaking, you won't feel confident enough to speak. Find a trustworthy friend or tutor with whom you can converse in English, who knows English, so that he can correct you reliably if required. Speaking fluent English may not be about having a vast vocabulary, but it definitely means having confidence that your message is received correctly by the listener and that you understand what the other person is trying to say. You can always look up words in the dictionary and keep increasing your vocabulary day by day.
But most important is practice, practice and practice!

When you speak English, read English magazines or books, watch English movies or TV series and listen to English news, you are around the language for most of the time. Then you start getting the hang of the word, and gradually you become fluent in it.
For thinking, you have to make additional efforts on your thoughts. Whatever your thoughts are, become aware and try to make an effort to start articulating in English. Although, it doesn't matter much if you think in your native language and can translate it effortlessly in English.
Any of these things take time, and you can develop it only with lots of practice.

English consists of grammar and vocabulary.
For both of these, you could start reading books that interest you preferably of a different genre each time so you can get a variety of words. When you get stuck on some words, immediately look at its meaning and synonyms.
Try to learn at least 1 or 2 new words each day and practise using them in sentences for the entire day.
If you get stuck with grammar, learn the correct usage from any app or grammar books or join a good class.
You can also watch English movies and TV series for understanding more usage, accent and flow of words.
Lastly, learning doesn't last without practice. So, practice a lot with the new knowledge of grammar and words.
