Along with reading and writing, you have to practice speaking in front of people, you can try with your family members, friends, etc. Try to implement new words while speaking which you will learn in your daily basis.

Very difficult question to answer, because we don't know at what level one is, the heights of perfection that one is desiring, is to just enable one to communicate, or to create an impact on the world ????
