Hope you are doing great!.
I read about your concern, and I completely understand the situation as I have also gone through the same. The only things which will help you in improving the spoken skills will be reading more n more books (content which includes conversation, i.e. conversation between 2 or more people) this will help you to understand how to respond.
Moreover, you can try using simple or basic English one word or two words while communicating with your family members(like food water bring and many more words).
I tried the same, now my mother speaks almost 30 % of English words with me and understands 80% of the language.
It will definitely work. Try it.
