It's not big job if he is getting 95 percent just ask him to do practice as much as he can. Don't put pressure on him to score 100.He will got 100 marks if he will not take any pressure during exam.

make him solve all the textbook questions (exercise, miscellaneous, examples) 2 or more times. Read tge textbook to be thorough with the concepts. If better conceptual knowledge is acquired, it is as simple as lifting a feather to score 100 in cbse 12th board exam. All the best.
