You are having good domin experience. So if you want to start your carrier as SAP MM Consultant all you have to do is start. Start learning and commit yourself that you will become consultant and work towards that. When the oppertunity comes you will be considered based on your knowledge and domain experience.
About English, as long as you are having passion towards anything nothing is impossible. There are may ways available to learn English. But how much time you are spending per day towards learning that matters.
