I want to give the solutions of your problems. Although I can not wirte everything but I try my level best to give you few tips.
i. Please study Prefix and Suffix on regular basis. Only one prefix or suffix will help you to learn many words.
ii. Please make your grammar strong.
iii. Read newspaper loudly and regularly.But you need guidance to choose the right newspaper or periodicals.Reading enhances the personality.
iv. If you want to be a good speaker than please be a good listner first. Listen any english program. Which english program should be watched or to listen , you need guidance.
v. Please interact with good trainer on regular basis.
vi. Concentrate on your body language, content knowledge pronunciation, rate of speach, voice modulation, presentation etc. For all these you need guidance.

I would like to give the solutions of your problems. Your problems are vocabulary, to memorise all the contents of topic and confidence to read either books or newspapers and take out the words and then cram all of them, is little bit difficult, but a good practise. I appteciate it. But it is boring and need regular revision. So, in my opinion the best solution is to study 'Prefix' and 'Suffix'. With the help of one suffix or prefix you will be able to memorise many words. And you can also prepare for compitive exams. Although I can not write everything but I can also give you few tips whuch can augment not only your Spoken Englush' but also help you to enhance your Confidence. Presentation, Body Language, Content Knowledge, Implementation of knowledge, Voice Modulation and other skills. So, finally my friend in short, I would request you to start with good guidance.
