It depends on, what is your current level of English.
The only exercise you can do which will give you visible results is "Practice Speaking with Delayed Subtitle". That means, download any Hollywood movies with subtitles. Now in "setting" delay the subtitles for 3 seconds (that means now subtitles will appear on the screen after 3 seconds).
There are 3 advantages of this exercise:
1. You can see visuals
2. You can hear the dialogue (in native style)
3. After 3 seconds you will speak the same dialogue when subtitle will appear on the screen.
Give yourself 100 hours challenge and I can guarantee you will observe significant improvement.

First, you need to identify what will work for you. Every individual has the different way of learning and understanding.
You can consider followings:
i. Content is the key (know what you've to speak)
ii. Speak with unknown people.
iii. Try to talk to Authoritative people.
iv. Listen to understand, no to reply.
v. Learn to "pause" before speaking.
vi. Face more and more real life situations (as these situations are extempore).
vii. Keep yourself motivated. Fail over and over and over again.
List is endless so identify what is your "Area of Improvement".
