MTI is one of the language barriers which makes one less confident and inferior than the others. Sometimes, it happens that while speaking people, one is so much into having a regional touch in their sounds. Generally, this happens because of a lack of exposition to any new language except one or two languages which is seen among older age people who are less flexible.
But it can certainly be changed if one gets trained and observed how he or she pronounces the words. For that, you can record your voice and can listen to it.

I believe that your work on students and their subsequent result speaks a lot. This is the field where consistently one needs to prove herself/himself as a support system to bring high scores as per candidates' requirement. This will create a conducive atmosphere for you to get a lot of leads.
Secondly, you can associate with any organisation those who bring the paid leads.

IELTS and OET both are test for language proficiency which means both are measuring alltogether same skills such as - listening, reading, writing and speaking. As far as level of the test vis a vis Nursing profession, i believe OET is much easier as the background of the test is profession specific on the contrary IELTS, having acdamic field such as -humanity, sst, history, science tech. Otherwise, all the fudnamental sneabling skills are same in both of the test. But, personnaly speaking as a trainer i wold recommand one to appear for OET if he or she is in medical profession, as it inculcate basic ethics of working.

There are certain parameters on which one can judge what is good or bad coaching. 

First of all trainer should be self motivating and dedicated towards his work. And personal one on one sessions are extremely importate for this. These sessions can be either on Skype or classroom. But I guess Skype is much better. 

Second thing is authentic resource to get prepared.

Third thing there should be flexible timings given by institution so that the best of time you can utilize in training sessions.

Last but not least take the demo and decide.

Ielts is based on fours fundamental enabling skills such as - listening, speaking and writing. 

It's not a big deal to crack this test, but it required some sort of guidance that will help in order to do away with language  problems. 

In information era you can't trust authenticity so the best way is to meet the trainer on Skype and take a demo session so that you can judge whether if he us going to help you.
